Surviving spouses of eligible First Responders who died in the line of duty should generate a First Responders Validation ID by entering the following information on behalf of their deceased spouse: First Responder Type, Organization, Full Legal First and Last Name and Date of Birth (Month / Day / Year). The email address of the surviving spouse should be entered to receive validation. Additional supporting documentation such as a copy of the death certificate and a letter from department human resources verifying the loss of life while on the job as either a Police Officer, Fireman or EMT may be required.
